Athletic Forms

All student-athletes are required to complete necessary paperwork prior to the start of their season. Forms can be found in the FrontRush roster portal. If you are getting a physical conducted by your own doctor, please use the form below. If you do not have insurance, please get the affadavit of no insurance notarized. For additional questions, please see the athletic trainer in Commons Building Room 120.
